Every three years, a regional community health needs assessment is performed in the North Central Washington region in an effort to understand the health needs of the communities in this area and to provide direction for the healthcare organizations, community hospitals, public health districts,and community organizations to focus their collaborative efforts on improving the health of our communities.
A community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. A plan is typically updated every three to five years.
